TBN WAIVER FORM, Release and Idemnification

Note:

This waiver is for adult registration for TBN events, including adults registering and participating at TBN events and who are bringing minors for which they are a parent or legal guardian.

Minors aged 16 or 17 can participate in TBN events on their own, provided that the parent or guardian files a waiver with TBN in advance. Click here for more information.

I hereby release and forever discharge the Toronto Bicycling Network Inc. (TBN), its officers, directors, servants, agents and other representatives ("Released Parties") from any and all claims, demands, damages, costs, expenses, actions and causes of action, whether in law or equity, in respect of death, injury, illness, loss or damage to my person, the participant undersigned for whom I am the parent or legal guardian (the “Participant") or property howsoever arising as a result of my participation or participation of the Participant in this activity or event organized and/or sponsored by the TBN. I understand that this release includes the release of all claims, demands, and causes of action which may arise by reason of any intentional act, negligence, gross negligence, error or omission on the part of the Released Parties. I declare that this release is binding upon me, my heirs, executors, administrators and assigns and those of the Participant.

I further undertake to hold and save harmless and agree to indemnify the Released Parties from and against any and all liability incurred by any or all of them arising as a result, or in any way connected to, my or the Participant’s participation in this activity or event organized and/or sponsored by the TBN.

I warrant that I am at least 18 years of age, that I am (or the Participant is) physically fit to participate in this activity, that, with respect to cycling events, my equipment or the equipment of the Participant is mechanically fit and suitable for its intended use in such activities, and that I or the Participant shall heed all traffic laws and wear a CPSC, CSA, SNELL, or ANSI approved helmet while riding a bicycle with the TBN.

Photo Release

I agree to allow TBN to use my (and/or the Participant's) name and photographic image in TBN publications, social media, on slide shows, the website and any other internal or external communications medium.
